EQUIPMENT USE AGREEMENT

Date of Drop-Off: Thursday, April 24, 2025

Date of Pick-Up: Saturday, April 26, 2025

Event: The Power of Conference – Confessional Booth

Lending Party: Justraw Creatives / The Church Collection 

Receiving Party: AME Church & AME Zion Church (Confessional Booth Unit)

Equipment Being Provided:

1. 2021 iPad Mini (with charging cable)

2. Selfie Lighting Set with Tripod

3. External Microphone Unit (connected to iPad)

Terms and Conditions of Use:

1. Purpose of Use: Equipment is being provided solely for the operation and engagement of the Confessional Booth at The Power of Conference.

2. Care & Custody:

   • The receiving party accepts full responsibility for the care, security, and safeguarding of all listed equipment from the time of drop-off to the time of pick-up.

   • The equipment must remain in the direct custody of the designated user(s) at all times and not be left unattended in public or unsecured spaces.

3. Access Security:

   • A lock code or passcode will be provided for access to the iPad.

   • This code must not be shared or distributed under any circumstance. Any unauthorized use resulting from a code leak will be the responsibility of the receiving party.

4. Loss, Theft, or Damage:

   • The receiving party shall be held financially liable for any loss, theft, or damage to any item listed above.

   • In the event of technical issues that occur due to negligence, misuse, or improper handling, the receiving party agrees to cover the cost of repair or replacement.

5. Return:

   • All equipment must be returned in the same condition it was received, with all parts and accessories included.

By signing below, the receiving party acknowledges and agrees to the above terms, and accepts full responsibility for the items listed under the conditions outlined.
