GAME EQUIPMENT USE AGREEMENT

Games and Equipment Being Provided:

1. Building Blocks (Jenga-style)

2. UNO No Mercy

3. Connect 4

4. Netflix (pre-logged in on provided device)

5. Stacking Game (Tetris-style)

Terms and Conditions of Use:

1. Purpose of Use: Equipment and games are provided exclusively for recreational use during Breathe Worship Social Night.

2. Care & Custody:

   
   

   • The receiving party assumes full responsibility for the care, use, and safeguarding of all items during the event.

   • Games and devices should be handled with care and kept in a supervised area during use.

3. Device Use & Access:

   
   

   • Any device provided with Netflix pre-logged in must remain secure.

   • Login information must not be altered or shared.

4. Loss, Damage, or Misuse:

   
   

   • The receiving party is financially responsible for any lost, stolen, or damaged equipment.

   • If an item is returned broken or incomplete due to misuse or negligence, reimbursement for replacement will be expected.

5. Return:

   
   

   • All items must be returned in the condition they were received, including original boxes or containers where applicable.

   • Inventory check will be conducted at return.

By signing below, the receiving party acknowledges and agrees to the above terms, and accepts full responsibility for the items listed under the conditions outlined.
